Application Forms

Nearly all applications (see exceptions below) for the OAC's Fiscal Year 2007 (activities for July 1, 2006 through June 30, 2007) must be submitted via OLGA, the OAC's web-based application system.

You must read Guidelines before applying to any OAC program. To see a list of all FY2007 deadlines visit the Deadlines page in Guidelines.

The Ohio Arts Council is looking for people to serve on advisory panels to review grant applications. To nominate a panelist complete a Nomination for Panel Membership form or contact Karine Aswad at karine.aswad@oac.state.oh.us or 614/466-2613.

Only forms for Arts Learning Artist in Residence—Artists (formerly Form C—Artists), Contract Summary will remain in the electronic versions (links below) for FY2006.

Final Report forms for FY2005 Applications will remain online until January 31. 2006.

Please read this page if you are applying for remaining funds for FY2006 funds in the following program: Arts Learning Artist in Residence—Artists (formerly Form C—Artists) or Contract Summary for use when contracting an artist listed in the Ohio Artists on Tour Directory. Links to pages containing the forms are below. You must link to a second page, based on whether you are a PC or Mac user, to access the download for the forms.

Using the Remaining Ohio Arts Council Electronic Applications

As the fonts used in the forms are Times New Roman - 12-point type, you may want to use this font for consistency.

You must print the application, sign it, and submit it to us. We must have original signatures on the application form submitted to us. We cannot accept applications submitted to us on disk or via e-mail.

For programs with deadlines, signed applications accompanied by all appropriate support materials must be in the OAC office no later than 5 p.m. on the deadline date. When a deadline falls on a weekend or holiday, the OAC will accept applications until 5 p.m. on the next business day.

The following forms will remain separate from OLGA. Please continue to use the executable or PDF versions of these forms:

  • Form C-Artists (Arts Learning Artist in Residence)
  • Contract Summary (OAOT)

The following Final Report Forms are available in this section of the site ONLY FOR FY2005 Applications:

  • Final Report (For Organizations)
  • Appalachian Arts Final Report
  • YouthReach Ohio Final Report

All the above forms are available in two versions  (Executable and PDF).

Please Note: The RTF forms for MAC users are no longer available for download from this site. If you are a MAC user and you are experiencing trouble with the PDF form please email webmaster@oac.state.oh.us.

  1. Executable files. (Approximately 500K) The files were created in OmniForm. Use these application forms if you are using Windows NT, Windows 2000, Windows ME, Windows 95 or Windows 98. These are program files and you must open them from your desktop.  Do not attempt to open them with Word or any other word processing application. 
  2. PDF files. Macintosh users should download and complete the forms created in the Adobe Acrobat Portable Document Format (PDF).  The applications in this format are identical to the executable forms created in Omniform but have some limitations. IMPORTANT!  Due to a limitation in Adobe Acrobat Reader, you can fill out the form on-screen and print the result, but you cannot save the completed document to your computer after it has been filled out.

Current Event Article

The Columbus Metropolitan Library, Ohio Arts Council and Friends of the Library have once again partnered to bring arts opportunities to artists and the public by presenting a juried art show at the Columbus Metropolitan Library Main Library. In 1998 the two organizations collaborated to create the Ohio Online Visual Artist Registry (OOVAR), a database that showcases the work of more than 700 artists from Ohio and around the world.
